FEBRUARY ISSUE OF TRANSPLANT TRIAL WATCH AVAILABLE NOW
February 2012 issue of the Transplant Trial Watch available now

To keep the transplantation community informed about recently published level 1 evidence in organ transplantation ESOT and the Centre for Evidence in Transplantation have developed the Transplant Trial Watch.

The Transplant Trial Watch is a monthly overview of 10 new randomised controlled trials (RCTs) and includes a brief description of each RCT, an assessment of the reporting of some methodological quality features and the main conclusions of the report written by the Centre for Evidence in Transplantation (CET). RCTs are selected by the Centre for Evidence in Transplantation on the basis of quality or interest. Electronic publications ahead of print are listed as [record in progress].

The first issue of the Transplant Trial Watch was published in September 2009. For all published RCTs in organ transplantation from 1970 and selected systematic reviews from 2008 go to the Transplant Library by logging onto the member avenue using your username and password.

Professor David Price: 1954 - 2012
The ELPAT secretariat is sad to announce that Professor David Price, Professor of Medical Law at De Montfort University (DMU), United Kingdom, has died after a short illness.

Professor Price, 57, who started his career at the university in 1977 as a lecturer in law, was an acknowledged expert on the legal and ethical aspects of organ donation and a founding member of ELPAT.

Professor Price wrote extensively on the law and ethics relating to transplantation and the use of human tissue and was a member of the Government’s Organ Donation Taskforce, which investigated the potential impact of an opt-out system for organ donation in the UK in 2008. In 2010, he joined the Nuffield Council on Bioethics working party, examining the issues around the donation of organs and other bodily material. Professor Price was the only legal representative on the working party that published its report, Human Bodies: donation for medicine and research, in October 2011.

With his knowledge, and since its establishment, David helped ELPAT move forward. He was a highly valued and respected member. His efforts and contributions will never be forgotten and the effects of his loss will be felt. It was always a joy to have him at our meetings.

David Price leaves a wife, Arlene, two children, Alex and Ben, and four grandchildren. Our sincere sympathy goes out to them.

We will miss him greatly.

Willem Weimar, ELPAT Chair

In Memoriam - Professor Gil Thiel
Dear ESOT members,

Unfortunately, I received the very sad news of the death of Gil Thiel.
Professor Thiel was one of the first to recognize the importance of living donors in kidney transplantation community in Europe. Gil Thiel was pensioned some years ago after many years as the head of nephrology and kidney transplantation in the University hospital in Basel, Switzerland. After his retirement he carried on taking care of the Swiss living donor health registry which he had founded in the 90’s – during his professional life he was always a pioneer for new approaches and the mentor of many nephrologists and transplant physicians in Switzerland
He had many contacts with European transplant specialists, had been active in the ESOT particularly as the Swiss representative for the Hesperis course.
He will be sadly missed.

Carla Baan,
President of ESOT

ESOT endorses the ISTANBUL DECLARATION
ISTANBUL DECLARATION

We, the representatives of the European Commission, the Organisation for Economic Cooperation and Development, the Organisation of the Islamic Conference, the United Nations, the United Nations Development Programme and the World Bank,
recognise that while our societies have become more complex, they are more closely linked than ever. Yet they retain differences in history, culture, and in economic and social development.
We are encouraged that initiatives to measure societal progress through statistical indicators have been launched in several countries and on all continents. Although these initiatives are based on different methodologies, cultural and intellectual paradigms, and degrees of involvement of key stakeholders, they reveal an emerging consensus on the need to undertake the measurement of societal progress in every country, going beyond conventional economic measures such as GDP per capita. Indeed, the United Nation’s system of indicators to measure progress towards the Millennium Development Goals (MDGs) is a step in that direction.
A culture of evidence-based decision making has to be promoted at all levels, to increase the welfare of societies. And in the “information age,” welfare depends in part on transparent and accountable public policy making. The availability of statistical indicators of economic, social, and environmental outcomes and their dissemination to citizens can contribute to promoting good governance and the improvement of democratic processes. It can strengthen citizens’ capacity to influence the goals of the societies they live in through debate and consensus building, and increase the accountability of public policies.
We affirm our commitment to measuring and fostering the progress of societies in all their dimensions and to supporting initiatives at the country level. We urge statistical offices, public and private organisations, and academic experts to work alongside representatives of their communities to produce high-quality, facts-based information that can be used by all of society to form a shared view of societal well-being and its evolution over time.
Official statistics are a key “public good” that foster the progress of societies. The development of indicators of societal progress offers an opportunity to reinforce the role of national statistical authorities as key providers of relevant, reliable, timely and comparable data and the indicators required for national and international reporting. We encourage governments to invest resources to develop reliable data and indicators according to the “Fundamental Principles of Official Statistics” adopted by the United Nations in 1994.
To take this work forward we need to:
• encourage communities to consider for themselves what “progress” means in the 21st century;
• share best practices on the measurement of societal progress and increase the awareness of the need to do so using sound and reliable methodologies;

• stimulate international debate, based on solid statistical data and indicators, on both global issues of societal progress and comparisons of such progress;
• produce a broader, shared, public understanding of changing conditions, while highlighting areas of significant change or inadequate knowledge;
• advocate appropriate investment in building statistical capacity, especially in developing countries, to improve the availability of data and indicators needed to guide development programs and report on progress toward international goals, such as the Millennium Development Goals.

Much work remains to be done, and the commitment of all partners is essential if we are to meet the demand that is emerging from our societies. We recognise that efforts will be commensurate with the capacity of countries at different levels of development. We invite both public and private organisations to contribute to this ambitious effort to foster the world’s progress and we welcome initiatives at the local, regional, national and international levels.
We would like to thank the Government of Turkey for hosting this second OECD World Forum on “Statistics, Knowledge and Policy.” We also wish to thank all those from around the world who have contributed to, or attended, this World Forum, or followed the discussions over the Internet.

DEATH OF DR BERNARD LOTY
ESOT Council is sad to announce the death of Dr Bernard Loty who passed away peacefully on Monday 7 June in Paris surrounded by his family. Dr Bernard Loty MD PhD was the Deputy Director for the French ‘Agence de la Biomedecine’ and was Chair of the Committee of Experts and Chair of the Steering Committee within the European Directorate for the Quality of Medicines and Healthcare, Council of Europe. Prior to these administrative posts, he was an orthopaedic surgeon who had over 20 years of experience in orthopaedic surgery and tissue banking. Although initially known amongst those involved in tissue transplantation, he gained stature and responsibility for the whole field of organ and tissue transplantation in Europe.

Bernard Loty was born in 1953 in Paris. He completed his studies at the Faculty of Medicine Xavier Bichat in 1985 and chose to move to orthopaedic surgery. During his work as an orthopaedic surgeon at the Cochin Hospital he became interested in the field of preservation of bone and was among the founders of the Association for the Study of Transplants and Substitutes in Orthopaedic Tissues. He chaired this organisation from 1990 to 1995.

In the late 1990’s, he became more involved in European administration in transplantation and played a vital role in the formation of the EU Directive on Tissue Cell and Gametes which is now used to regulate activities in this area across Europe. In 2005 Bernard Loty became Medical Director and Assistant Executive Director of the Biomedicine Agency in France.

He was married to Dominique Loty, Lawyer at the Paris Bar and was father to 4 children. ESOT Council sends it condolences to the wife and family of Bernard Loty and commemorates his significant input into European transplantation.

Dr. J.A. Hutchinson, MBBChir PhD, WINNER of the ASTS-ESOT Exchange Grant 2009
We are proud to announce the winner of the ASTS-ESOT Exchange Grant 2009. Unanimously Dr. J.A. Hutchinson, MBBChir PhD, is selected by the Grant Committees of ASTS and ESOT to be awarded the ASTS-ESOT Exchange Grant 2009.

Investigator: Dr. J.A. Hutchinson, MBBChir PhD
Affiliation: Klinik und Poliklinik für Chirurgie
Universitätsklinikum Regensburg, Bayern, Germany
Host institution: Department of Surgery, University of Wisconsin U.S.A.

Synopsis: EVALUATION OF THE EFFICACY OF REGULATORY MACROPHAGES AS A TOLERANCE-INDUCING STRATEGY IN A CANINE RENAL TRANSPLANT MODEL

Conditioning the response of transplant recipients to donor alloantigen using cell-based therapies is now becoming a clinically feasible strategy and, as the potential risks are better understood and minimised, such approaches are gaining credibility. Several alternative cell types are now approaching the point of preclinical development that would allow them to be properly investigated as adjunct immunosuppressive therapies in early-stage clinical trials, including regulatory macrophages (M reg).

By its mode of derivation, cell surface phenotype, gene expression profile, cytokine secretion and potent T cell-suppressive potential, the human M reg has been shown to be a macrophage in a novel state of activation, distinct from all previously described states of macrophage polarisation. Owing to its phenotypic stability, robust nature, terminally-differentiated state and low immunogenic potential, the human M reg may be particularly suitable for use as a tolerance-inducing therapy in transplantation.

The objective of the work to be funded by the ESOT-ASTS Exchange Award is to assess the capacity of M regs to induce a state of donor alloantigen-specific hyporesponsiveness in a canine renal transplant model. This will entail the derivation of a canine equivalent of the human regulatory macrophage and the establishment of techniques allowing the generation of canine M regs in sufficient number and quality to be used in transplantation studies at the University of Wisconsin.

• encourage communities to consider for themselves what “progress” means in the 21st century;
• share best practices on the measurement of societal progress and increase the awareness of the need to do so using sound and reliable methodologies;

• stimulate international debate, based on solid statistical data and indicators, on both global issues of societal progress and comparisons of such progress;
• produce a broader, shared, public understanding of changing conditions, while highlighting areas of significant change or inadequate knowledge;

HANS BRYNGER, Co-Founder of ESOT - In Memoriam
ESOT joins colleagues and scientific organizations in honouring and mourning the loss of Professor Hans Brynger, M.D., Ph.D., a distinguished transplant surgeon and clinical researcher.

Hans Brynger is well known as a pioneer in the field of organ transplantation and was one of the cofounders of the European Society for Organ Transplantation.

Hans was born in the northern part of Sweden in 1936 and started his academic education in 1955, studying odontology in Stockholm. In 1956 he switched to medical school at the University of Gothenburg. He was very actively involved in various student activities, such as writing plays for the students and playing in the student orchestra, skills he shared with his friends later in life. Some of his closest friends in later life were actually student fellows from this period.

From 1965 on he took his first steps as a young general surgeon in two county hospitals outside Gothenburg, and in 1970 he started his career at the Sahlgrenska University Hospital at the Department of Surgery I. During these years he also married Britt and the young couple was soon blessed with two twin boys born in 1964 and a daughter born in 1966. At Sahlgrenska he teamed up with Professor Lars-Erik Gelin (also called “Charlie” by his friends). Here he got his training in transplantation surgery which at that point was in the really early days. Lars-Erik Gelin had a broad network of international collaborators among the pioneers of kidney transplantation and Hans was introduced to this network. In 1975 he finished his Ph.D. studies on experimental pancreas transplantation in dogs, and a few years later was appointed Associate Professor of Surgery. Already early in his clinical career, in 1972 or1973, he also performed clinical pancreas transplants.

When Professor Lars-Erik Gelin died in 1980, Hans became the Head of the Transplant Unit. At that point Sahlgrenska University Hospital had one of the largest and most experienced transplant units in Europe and in the world, performing some 150 kidney transplantations, 15 pancreas transplantations, and 20-30 liver transplantations a year.

In 1981 the first Lars-Erik Gelin memorial symposium on clinical kidney transplantation took place in Gothenburg. During and after that meeting Hans Brynger, Raimund Margreiter (Innsbruck), Georg Uhlschmid (Zürich), Walter Land (Munich) and Gerhard Opelz (Heidelberg) discussed the idea of a European Society of Organ Transplantation. They all became co-founders of ESOT, and in 1987 Hans hosted the third ESOT Congress in Gothenburg. Professor Brynger served on the editorial board of several international journals, like Transplantation Proceedings, Nephrology, Dialysis and Transplantation, Clinical Transplantation and Transplant International. He also was invited as lecturer in countries all over the world.

Hans encouraged many young medical students to start a career in the field of transplantation and was the main scientific tutor of several Ph.D. students. He always had a good way of encouraging young students when they needed it most. All the way through his career he also followed the tradition of Lars-Erik Gelin, characterized by enthusiasm and humour within the department.

Professor Brynger ended his professional career as a transplant surgeon in 1990 when he took on another career being responsible for “foreign affairs” regarding patients, post graduate training and projects in health care planning and management within the fields of medicine and odontology. Parallel with this activity he still maintained contact with many of his international colleagues and was frequently invited as a lecturer.

Hans will be missed and remembered by all his colleagues at the Transplant Institute at Sahlgrenska University Hospital as well as by all of his international colleagues in the field of organ transplantation.

Brief Report on the International Congress "GIFT FOR LIFE. CONSIDERATIONS ON ORGAN DONATION", held in Rome, 6-8 November 2008
The Pontifical Academy for Life (PAV), the Italian National Transplant Centre (CNT) and the World Federation of Catholic Medical Associations (FIAMC) organized an international Congress on organ donation entitled: “A gift for life. Considerations on organ donation”, in Rome, last November 6th to 8th at the Auditorium della Conciliazione, nearby the Vatican City.

The Congress focused on the issue of organ donation which was presented by scientists, researchers and ethicists as well as Catholic Associations from all over the world. During the Congress scientific, legal, ethical and organizational aspects of transplants and donation activity in the five continents were widely discussed.

The first day of Congress introduced the current transplant system on an international basis focusing specially on the historical evolution of both transplant sciences and organ donation. The crucial and alarming aspect of organ shortage, traffic and organ tourism was also deeply debated. The second day of Congress was dedicated to ethical and anthropological aspects on organ donation as well as to all issues related to the organ-donation-living will, organ allocation criteria and ethics of the transplant organization. Of a particular interest were training paths and pastoral initiatives promoting organ donation culture.

During the second day of the event Congress participants were received by the Holy Father Benedict XVI who presented the Church’s position in favour of organ donation and transplants, defining donation as an admirable act, not just morally licit, since it expresses a genuine testimony of charity. The Holy Father moreover underlined the need to respect all conditions that make organ donation and transplantation legal and to fight any kind of coercion or organ commercialization. At last the Holy Father urged researchers and scientists to increase research in order to dispel people’s “prejudices and misunderstanding” and disperse their “mistrust and fear”, replacing them with certainty and guarantee.

The final session of the Congress, dedicated to the role of mass media in promoting organ donation culture and in influencing people’s perception of it, outlined the need to build an alliance between the world of transplants and media in order to promote an ethical communication oriented to increasing the actual knowledge of this field among people through a correct information, keeping them at a safe distance from a superficial and sensationalistic use of news.

Among the over 500 people registered at the Congress there were: health and pastoral professionals, donors’ and patients’ associations, Directors of national transplant systems, journalists and patients. All Italian and various international media attended the meeting with the Holy Father Benedict XVI and published news from the Congress and the reflections the Holy Father expressed on the subject.

Eight years after the international Congress on transplants in 2000, the Congress “A gift for life. Considerations on organ donation” represented an important occasion to build up a global state of the art on organ donation and transplant. The success of the Congress in terms of people registered and media coverage is mainly due to the precious words of the Holy Father and the organizing contribution of the Pontifical Academy for Life (PAV) and the World Federation of Catholic Medical Associations (FIAMC) in generating ethical and cultural considerations on organ donation.

NEW: The TRIPLE “C” Course on Deceased Donor Co-ordination, Living Donor Co-ordination and Clinical Co-ordination
The European Donation Committee of ESOT (EDC) is preparing an advanced course in organ and tissue transplantation. This new ESOT course, called “TRIPLE C”, will be launched in 2009 centered on the three professional coordinator groups of donor coordinators, recipient coordinators for living donors and recipient coordinators for cadaveric donors. The objective of this course is to enable expertise and knowledge in the field of donation to be promoted and exchanged. With this course the EDC aims at a further deepening of topics which are not addressed in the majority of courses in this field.

Aim
The TRIPLE C Course is an advanced course in donation and transplantation for co-ordinators and is organized under supervision of the European Donation Committee (EDC) of ESOT. The course aims to provide insight and tools in addition to existing educational programmes on donation on a local/regional or national level.

Main Topics
The TRIPLE C course would focus on “public relations management” as well as “in house management” of donation and transplantation process and scientific research education. The format and methodologies of this course would be an interesting analysis of senior coordinator.
Main topics of the course would be:
- European legislation and guide lines
- Communication and donor’s relative relationship
- Management (quality management,
HR management)
- Clinical risk management
- Planning and organization of donation and transplantation process
- Scientific education and research project

Target
The course focuses on three professional coordinator groups of donor coordinators, recipient coordinators for living donors and recipient coordinators for cadaveric donors:
- Deceased Donor Co-ordinators (DDC),
- Living Donor Co-ordinators (LDC)
- Clinical Transplant Co-ordinators (CTC)

To be eligible for the course, these professional should have an accomplished basic training in their function, a current position as co-ordinator, at least three years of experience as co-ordinator.

Application procedure
Candidates apply for participation using the ESOT website:
- uploading their application form and CV,
- uploading a presentation letter of their supervisor,
- providing a statement of good comprehension of the English language
- three statements related to the topics of the course (communication, risk factors and/or management issues in donation or transplantation).

Selection procedure
The maximum number of participants will be 40-50 (including all three categories of co-ordinators as defined in the target group). The distribution over the categories will be in proportion to the average number of co-ordinators working in that specific area. As ESOT represents all European countries, a fair selection system will be used similar to the one used by the Education Committee for other ESOT courses with a fixed number of seats in relation to transplant activity per country/region will apply. Final selection of the participants will be made by the members of the EDC.

Organization and cost
The Triple C Course will be organized once every year by a host in collaboration with his national organization in one of the European countries represented by ESOT. The annual course will be co-hosted by the organizer of the course of next year.
The duration of the course will be 3 days.
The costs for the course will be covered by ESOT, except for a fixed fee of € 150 and travel cost, both to be paid by the participants.

Dates and Venue
First edition of the TRIPLE C course would be in early 2009, in Germany

Further information will be published soon on the ESOT website.

NEW: Surgical Technique of the Abdominal Organ Procurement, Step by Step, by Andrzej BARANSKI
Prominant ESOT member and member of the Education Committee Dr. Andre Baranski from the Leiden University Medical Centre has written a very instructive and complete book which focuses on one of the key topics in organ donation and transplantation: The quality and safety of organ procurement. As ESOT enhances training and certification in organ donation with a masterclass (EDSM) and a new e-learning module, we would like to attract your attention to this distinguished and important product of one of our ESOT members.

At present, most multi-organ donor (MOD) procurement procedures are performed by a local surgical team, which is not necessarily the same team as the one responsible for the transplantation. The accepting center, therefore, has to rely on the expertise and skills of the procurement team. Adequate training and standardization of the surgical techniques is therefore of vital importance, and this book aims to aid both with the most up-to-date information.

Encompassing Dr Baranski’s 20 years of experience in the filed of abdominal organ donation, this book is a comprehensive step-by-step guide to the field, with many colour photographs and clear drawings to aid the reader. The text concisely demonstrates the many difficult steps in the organ procurement process, also highlighting the major potential pitfalls and common surgical mistakes that have arisen during the author’s extensive career.

The success of organ transplantation starts with the optimal medical and surgical treatment of the donor as without good organ harvesting, there is a high mortality incidence in the patients on the waiting list and thus less space for new patients. Organ procurement is therefore now recognized as an essential part of organ transplantation. In most donors, the surgical procedure can be challenging for an inexperienced surgeon and so often a detailed walkthrough is required to prepare for the operation. This part of transplant surgery is critically important as inadequate preservation or surgical injury at this stage can lead to complete organ damage or to serious complications in the recipient. At present, most multi-organ donor (MOD) procurement procedures are performed by a local surgical team, which is not necessarily the same team as the one responsible for the transplantation. The accepting center therefore has to rely on the expertise and skills of the procurement team, meaning that adequate training and a standardization of the surgical techniques is of vital importance.

This title is a useful pocket guide for practicing surgeons or senior residents participating in organ procurement teams, wishing to learn abdominal organ procurement. The high illustrative content offers great worth as a step-by-step walkthrough, with clear diagrams further supplementing the text to provide the most detailed handbook available on abdominal transplantation.
